Home Care Package: General Information
Home care Packages can be used for a range of services including Allied health (Physiotherapy), social services, gardening, cleaning, transport, personal care, respite care and more. Home Care Packages are divided into four funding levels. The higher the level, the more care hours are included. These are:
Level 1: 6.8 hours per week
Level 2: 12.6 hours per week
Level 3: 21.4 hours per week
Level 4: 27.5 hours per week
The funding level is determined based on an assessment of the patient’s condition and living circumstances. An assigned assessor will work with the client and their family to understand their needs and determine the most appropriate Home Care Package for them. The hours included in each level can be divided between the above mentioned services. Typically, a person will coordinate their care plan with their home care package provider and their needs.
Home Care Package Assessment
The first step in obtaining funding for a Homecare Package is to register at My Aged Care and complete an initial application here: https://www.myagedcare.gov.au/assessment/apply-online. Once completed, MyAgedCare will contact the applicant to discuss further. Next, an in-home assessment will be scheduled by the aged-care assessment team from the Aged Care Assessment Team (ACAT) service. The assessor will schedule time to visit the patient’s home and complete an assessment to determine whether or not a Home Care Package is an option and, if it is, what package level best meets the support needs of that individual. After the assessment, My Aged Care will send a letter to the patient to notify them if they are eligible for a Home Care Package and what level of funding they’ve been approved for. They will then be placed on a waitlist and will be notified once the package becomes activated. Waitlists can be variable as they are prioritised based on needs. Typically, a person will then choose a Home Care Package provider who will assist to organise their supports.

As an Australian, AHPRA registered Allied Health provider, people can use their Home Care Package funding to receive our caring services.
We also provide mobile Physiotherapy services to home care clients.
